We provide Service Coordination (Case Management) for individuals enrolled in — or seeking enrollment in — the NHTD Waiver and TBI Waiver programs.
What We Do
We specialize in Medicaid Waiver Case Management, helping participants:
1. Navigate NHTD and TBI Waiver eligibility
2. Coordinate medical, social, and community-based services
3. Develop and maintain person-centered service plans
4. Access housing, benefits, and community supports
5. Remain safely in the community trying to avoid institutional placement.
Our role is to coordinate, advocate, and monitor.
Participants direct their services; we help them make it happen.
